Liability Waiver Agreement
I understand that yoga and pilates includes physical movements as well as an opportunity for relaxation, stress reduction and relief of muscular tension. As is the case with any physical activity, the risk of injury is always present and cannot be completely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity and ask for support from the instructor. I take full responsibility for managing my own wellbeing during and after each yoga or pilates class and not over extend beyond my capabilities. I understand that yoga and pilates is not a substitute for medical attention, examination, diagnosis or treatment. Yoga and pilates is not recommended and is not safe under certain medical conditions. By booking in for a class, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program, if required. In addition, I will make the instructor aware of any medical conditions or physical limitations before class. If I am pregnant, my signature verifies that I have my physician’s approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and pilates and participation is at my own risk. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against My Yoga Space and its teachers and all related facilities and premises for any personal injury or negligence. Additionally, the instructor is not in any way responsible for any loss or damage of your personal property. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.
I have carefully read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am accepting this agreement voluntarily and recognise that my class booking serves as complete and unconditional release of all liability to the greatest extent allowed by law and that it cannot be changed orally.
I agree to these terms and policies.
Covid-19 Liability Waiver
If you attend classes at My Yoga Space, you agree to the following Liability Waiver:

I acknowledge the contagious nature of the Coronavirus/COVID-19.

I further acknowledge that My Yoga Space has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that My Yoga Space cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, studio staff, and other studio clients and their families.

I voluntarily seek services provided by My Yoga Space and I acknowledge the contagious nature of COVID-19. I voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not travelled internationally within the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with My attendance at My Yoga Space. By attending the studio I release, covenant not to sue, discharge, and hold harmless My Yoga Space, its employees, agents, and representatives, from any claims. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of My Yoga Space, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any class.
